How is a condition like mine usually treated?
First find the situation closest to your existing diagnosis. This section describes common treatment approaches; an individual plan also depends on pathology, tests and treatments already received.
Low-risk papillary microcarcinoma
Some patients may discuss active surveillance. China's 2022 guideline lists a single lesion smaller than 1 cm, centrally located in the gland, without regional lymph node metastases. A specialist still assesses observation suitability; size alone cannot decide.
Resectable differentiated cancer
Papillary and follicular cancers usually mainly involve surgery. Lobectomy, total thyroidectomy and lymph node dissection depend on lesion distribution, invasion and metastases. Final pathology determines subsequent treatment.
Recurrence, metastasis or radioiodine-refractory disease
First assess whether lesions can be resected again, take up radioactive iodine and are continually progressing. Some patients can have local treatment; others discuss targeted medicines. A single marker rise cannot directly justify changing medicines.
Medullary or anaplastic cancer
These cannot follow low-risk papillary cancer pathways. Medullary cancer needs consideration of RET and hereditary assessment. Anaplastic cancer may progress rapidly; airway problems and treatment assessment take priority.
02
Which records affect the plan?
Does ultrasound show a suspicious nodule, or is there a pathological diagnosis?
Ultrasound describes nodule size, location and cervical nodes. Needle or surgical pathology establishes nature and type. 'Suspicious for malignancy' is not the same as confirmed cancer. Submit the original report, not only an ultrasound grade.
- Records to check
- Thyroid and cervical node ultrasound, needle cytology or tissue pathology; keep examination dates and images.
- Questions for your doctor
- Do existing results establish the subtype? What would review or repeat sampling answer?

What determines lobectomy versus total thyroidectomy?
Doctors consider more than maximum diameter: unilateral or bilateral lesions, multifocality, extrathyroidal invasion, nodal and distant metastases, and anticipated iodine-131. Some differentiated cancers confined to one side may be discussed for lobectomy with isthmusectomy. Total thyroidectomy requires long-term hormone replacement.
- Records to check
- Ultrasound lesion location, existing imaging, pathological subtype and previous surgical extent.
- Questions for your doctor
- Why recommend this extent? If one lobe is removed first, which post-operative findings could lead to completion surgery?
Stage Ⅰ still requires post-operative assessment
Differentiated cancer staging depends partly on age. Doctors separately stratify recurrence risk. Margins, extrathyroidal or vascular invasion, and numbers and sizes of nodal metastases can affect iodine-131 and follow-up. One stage number cannot replace full pathology.
- Records to check
- Complete post-operative pathology, operation records and available thyroglobulin (Tg) and thyroglobulin antibody (TgAb) results.
- Questions for your doctor
- What is my recurrence risk group? Which findings support giving or omitting iodine-131?
Medullary cancer genetic tests: distinguish tumour and hereditary questions
RET results may inform medicines or indicate genetic counselling. Germline and tumour tests serve different purposes. A counselling or specialist team should explain whether relatives need tests after relevant findings.
- Records to check
- Pathology, calcitonin and CEA, available RET reports and family history.
- Questions for your doctor
- Does this test address treatment or inheritance? Is further family risk assessment needed?
03
Treatment options and timing
These regimens explain treatment components and timing; they are not individual prescriptions. A cycle includes recovery and assessment time and cannot be directly treated as the number of days in hospital or in China.
Active surveillance
Low-risk papillary microcarcinoma assessed by a specialist as suitable for observation, with reliable planned follow-up.
Active surveillance
Low-risk papillary microcarcinoma assessed by a specialist as suitable for observation, with reliable planned follow-up.
- Treatment details
- Regular assessment monitors changes; it does not use medicines to eliminate a nodule. China's 2022 guideline lists changes warranting renewed surgery discussion, including a 2 - 3 mm diameter increase, new lesions or suspicious metastatic nodes.
- Cycles and timing
- The guideline generally proposes reassessment every 6 months. The doctor confirms individual intervals and criteria for surgery in writing.
Observation reduces immediate surgical burden but needs reliable ultrasound follow-up and acceptance of possible future surgery. If living between countries, establish where tests occur and who compares results first.
Lobectomy or total thyroidectomy
Resectable thyroid cancer; extent depends on type, invasion, distribution and subsequent treatment.
Lobectomy or total thyroidectomy
Resectable thyroid cancer; extent depends on type, invasion, distribution and subsequent treatment.
- Treatment details
- Remove the primary lesion and manage cervical nodes when needed. Final pathology then informs completion surgery, iodine-131 or other treatment.
- Cycles and timing
- Plan outpatient assessment, surgical admission, pathology results and reassessment separately. Return travel depends on recovery and subsequent treatment.
Discuss voice, swallowing and low-calcium risks before surgery. Total or repeat surgery especially needs nerve and parathyroid protection discussion. Doctors assess fitness to fly.
Radioactive iodine (iodine-131)
Some post-operative differentiated cancers or iodine-avid lesions; use depends on risk and treatment goals.
Radioactive iodine (iodine-131)
Some post-operative differentiated cancers or iodine-avid lesions; use depends on risk and treatment goals.
- Treatment details
- Oral radioactive iodine treats relevant residual tissue or lesions. It is not used for medullary cancer. Total thyroidectomy alone does not make it mandatory for low-risk differentiated cancer.
- Cycles and timing
- Preparation, administration, radiation protection and reassessment are separate stages. Nuclear medicine staff explain admission or isolation and distancing from family for the individual arrangement.
Pregnancy and breastfeeding involve contraindications and protection requirements. Explain conception plans, breastfeeding and living with children before booking. Do not stop thyroid medicines yourself or book return travel solely by administration day.
Thyroid hormone replacement and TSH management
Hormone replacement is needed after total thyroidectomy. Some differentiated cancers also need risk-based thyroid-stimulating hormone (TSH) management.
Thyroid hormone replacement and TSH management
Hormone replacement is needed after total thyroidectomy. Some differentiated cancers also need risk-based thyroid-stimulating hormone (TSH) management.
- Treatment details
- Doctors adjust levothyroxine using risk, reassessment and health. Replacement and TSH suppression have different goals. Larger doses do not necessarily prevent recurrence better.
- Cycles and timing
- Long-term medication and reassessment are often needed. The 2022 guideline lists different TSH targets by risk. Put individual values and blood test dates in the follow-up plan.
Report palpitations, fatigue or other symptoms to the doctor. Do not change doses using website targets. Confirm medicine supply at home and keep names, strengths and instructions.
Targeted medicines or radiotherapy for advanced disease
Some continuously progressing, unresectable or radioiodine-refractory disease, with subtype and molecular results considered.
Targeted medicines or radiotherapy for advanced disease
Some continuously progressing, unresectable or radioiodine-refractory disease, with subtype and molecular results considered.
- Treatment details
- Specialists assess how local and systemic treatment connect. China's 2022 guideline lists sorafenib for appropriate radioiodine-refractory differentiated cancer. Other medicines require current Chinese product information and availability checks.
- Cycles and timing
- Oral treatment often continues with regular reassessment; no universal course ends after a fixed cycle count. Radiotherapy sessions depend on site, previous treatment and plan.
Targeted medicines have side effects and monitoring requirements. Starting times may differ for stable and clearly progressing disease. Clarify goals, monitoring and adjustment criteria first.
04
When to discuss a second opinion or MDT
Doctors disagree about surgical extent
Submit both opinions, ultrasound and pathology. Specify comparison of lobectomy and total thyroidectomy, possible completion surgery and subsequent medicines. Ask the doctor to explain the recommendation using your images and pathology.
Uncertainty about post-operative iodine-131
Ask thyroid surgery and nuclear medicine teams to discuss final pathology, residual disease or metastases. Questions can include goals, follow-up without treatment, and preparation and protection if treated.
Repeat surgery, neck invasion or recurrence
Provide previous surgical extent, voice changes, iodine-131 and radiotherapy records. Resectability assessment also needs airway, swallowing and repeat surgery risk discussions. Complex cases may request relevant specialties in an MDT.
05
New medicines and other treatment options
Can thermal ablation replace surgery?
Before discussing cancer ablation, confirm pathology, lesion location and nodal assessment. Ask why it is suitable, how local control is measured and how absence of a full surgical specimen affects subsequent decisions.
Does a positive gene result mean a suitable new medicine exists?
Read RET, BRAF and other results with pathology, progression and specific indications. NCI-listed medicines are not necessarily approved for your situation in China. Check current Chinese information, hospital supply and trial conditions with the team.
06
Voice, low calcium and follow-up at home after surgery
Voice changes need more than waiting for wound healing
Tell the surgical team about pre-existing hoarseness or choking when drinking after surgery. Ask about vocal cord assessment and management. Explain occupational voice needs before treatment. Nerve monitoring cannot guarantee an unaffected voice.
Calcium and hormone medicines need specific instructions
After total thyroidectomy, doctors monitor parathyroid function and calcium. At discharge, obtain instructions on supplementation, duration, blood tests and symptoms requiring contact. Do not attribute all fatigue to low calcium or increase doses long-term yourself.
Agree how local follow-up results will be compared
China's 2022 guideline suggests first post-operative neck ultrasound for differentiated cancer at about 3 months for high risk and 6 months for intermediate or low risk, sooner for suspicious lesions. This is not one schedule for every type. Before returning home, confirm how ultrasound, Tg/TgAb and thyroid function results reach the receiving doctor.

International department costs and budgeting
This page uses self-pay pricing for international departments / international medical services at Chinese hospitals. A verifiable published total price for a full course of care for Thyroid cancer has not yet been obtained. Ordinary outpatient, standard ward or insurance settlement amounts cannot be used as substitutes.
Available international department itemised charges
The following charges come from the International Medical Center at The University of Hong Kong-Shenzhen Hospital, published on 2025-06-04, using the amounts converted on 2026-10-12. They are references for itemizing a budget, do not represent prices at all hospitals and are not a package for Thyroid cancer.
| Item and scope | Reference amount (USD) |
|---|---|
| Specialist outpatient consultationPer consultation, with fees varying by doctor category. Tests, medicines and treatment are charged separately. | USD 90 - 300 |
| Adult inpatient consultationDaily consultation charge only. Beds, surgery, medicines and other charges are separate. | USD 150 - 220 |
| Single standard room bedPer bed per day, corresponding to Standard Room 1 and Standard Room 2 in the published schedule. Inpatient consultations and treatment are excluded. | USD 180 - 220 |
Converted using the reference exchange rate on 2026-10-12: USD 1 = CNY 6.6928, rounded to USD 10. Dollar amounts are for budgeting only. The hospital bills in its actual settlement currency; exchange rates and payment fees are separate.
Specify surgical extent in the budget first
Quotes should state lobectomy or total thyroidectomy, neck dissection and room type. Include or separately list pre-operative tests, anaesthesia, surgery, consumables, nerve monitoring, pathology and admission. Clarify charges for repeat surgery or extended admission.
Estimate iodine-131 separately from surgery
Check nuclear medicine assessment, preparation tests, medicine, dedicated rooms or isolation, imaging and reassessment separately. Companion accommodation and return changes are not hospital administration charges. One medicine price cannot represent the whole budget.
List long-term medicines and reassessment separately
Include hormones, necessary calcium, ultrasound and blood tests as ongoing costs. For targeted treatment, ask the international department to quote a specific name, strength and 30-day quantity, plus monitoring. 30 days is only a budget comparison unit, not a course.
SinoCareHub helps obtain an itemised estimate from the prospective receiving international department, specifying the plan, date, validity period, included items and separately charged items. Hospital medical charges, platform coordination fees and third-party costs such as transport and accommodation are explained separately. The budget is updated if the treatment plan changes.
08
Which records to organise first
- Thyroid and cervical node ultrasound and available needle or surgical pathology.
- After surgery: operation records, full pathology, dissected node results and discharge summary.
- Thyroid function and available Tg/TgAb; medullary cancer should include calcitonin, CEA and relevant genetic reports.
- Iodine-131 doses and dates, post-treatment imaging, prior radiotherapy or medicines and current medication.
First submit the records you already have and identify anything missing. The prospective receiving hospital decides whether additional tests or borrowing slides or paraffin blocks are needed for this assessment.
09
Common patient questions
Does BRAF positivity require total thyroidectomy?
One molecular result cannot determine surgical extent. Doctors also assess subtype, location, lesion numbers, invasion and nodes. Submit the complete report and ask which decision the result changes.
Must I collect all medicines in China after total thyroidectomy?
Long-term replacement is usually needed, but repeated China trips are not necessarily required. Check local supply, strengths and follow-up before discharge. The follow-up doctor adjusts doses.
Does one raised Tg result mean recurrence?
A single result cannot establish that. Interpret Tg alongside TgAb, TSH status, surgical extent, iodine-131 history and trends. Keep testing-method information and ask the doctor to assess imaging too.
Can I immediately live with my children after iodine-131?
Obtain specific nuclear medicine radiation protection instructions first. Contact with children or pregnant women, accommodation and transport may be affected. Other patients' experiences cannot determine when your restrictions end.
