How is a condition like mine usually treated?
First find the situation closest to your existing diagnosis. This section describes common treatment approaches; an individual plan also depends on pathology, tests and treatments already received.
Very early lesions confined to the bowel wall
Some lesions may be assessed for endoscopic or local resection. Complete removal depends on invasion depth, margins and other risks in full pathology. Bowel resection may still be needed after polyp removal.
Colon cancer without distant metastasis
Resectable disease often has bowel and regional node resection first, with chemotherapy decided from pathology. Stage Ⅰ usually needs no adjuvant chemotherapy; stages Ⅱ and Ⅲ require pathological risk assessment.
Locally advanced rectal cancer
Pelvic MRI and anal function affect sequence, particularly for mid- and low-rectal lesions. Some receive chemoradiotherapy or total neoadjuvant treatment before resection, sphincter preservation and stoma assessment.
Liver or lung metastases or local recurrence
Identify lesions amenable to resection or ablation and those needing medicines first. Metastasis does not automatically remove every surgical opportunity, but local treatment cannot replace systemic assessment.
02
Which records affect the plan?
Where is the lesion, and what remains after endoscopic removal?
Colonoscopy gives location; pathology establishes invasion and margins. Colon and rectal surgery and radiation differ. Incomplete endoscopic specimens may prevent adequate subsequent risk assessment.
- Records to check
- Colonoscopy, complete biopsy or endoscopic resection pathology and available abdominal and chest imaging.
- Questions for your doctor
- Has endoscopic removal achieved the goal? Which risk would further surgery address?

Rectal sphincter preservation involves more than distance from the anus
Assess relationships to sphincters, surrounding tissues and expected margins, plus existing continence. Preserving the anus does not guarantee unchanged bowel function. A protective stoma may accompany sphincter-preserving surgery.
- Records to check
- Original pelvic MRI and report, rectal examination and previous surgery or radiation records.
- Questions for your doctor
- What function is expected afterwards? Is a temporary stoma needed, and when might reversal be impossible?
What do MMR, MSI and genetic results change?
Mismatch repair (MMR) proteins or microsatellite status (MSI) relate to some immunotherapies and hereditary risk assessment. Metastatic medicines also consider RAS, BRAF, primary location and previous treatment. 'Normal genes' cannot replace the complete report.
- Records to check
- Available MMR immunohistochemistry, MSI, KRAS, NRAS, BRAF and other molecular tests with methods and dates.
- Questions for your doctor
- Which result affects this plan? Is genetic counselling or family risk assessment needed?
03
Treatment options and timing
These regimens explain treatment components and timing; they are not individual prescriptions. A cycle includes recovery and assessment time and cannot be directly treated as the number of days in hospital or in China.
Endoscopic or local resection
Some assessed early lesions, with a specimen allowing margin assessment.
Endoscopic or local resection
Some assessed early lesions, with a specimen allowing margin assessment.
- Treatment details
- Discuss endoscopic mucosal resection or submucosal dissection. Complete pathology then determines observation, repeat local treatment or bowel resection with node dissection.
- Cycles and timing
- Schedule examination, removal, pathology and follow-up separately. Completing one endoscopic procedure does not necessarily finish treatment.
Ask about perforation, bleeding and additional surgery. Request existing specimen review first to avoid unnecessary repeat procedures for a China trip.
Bowel resection, sphincter preservation and stomas
Resectable colon or rectal cancer, with extent determined by location, margins and health.
Bowel resection, sphincter preservation and stomas
Resectable colon or rectal cancer, with extent determined by location, margins and health.
- Treatment details
- Resect the lesion and nodes and assess anastomosis. A stoma may temporarily protect the join or remain long-term. Laparoscopic, robotic and open approaches alone cannot determine sphincter preservation.
- Cycles and timing
- Time admission, wound healing, eating and bowel recovery separately. Stoma reversal needs another assessment and possibly surgery; it is not automatically part of the first admission.
Clarify stoma possibility, learning care, supplies and reversal criteria beforehand. Quotes should state whether staplers, stomas and subsequent surgery cost extra.
Post-operative adjuvant chemotherapy
Need depends on pathological stage, risk, molecular results and recovery.
Post-operative adjuvant chemotherapy
Need depends on pathological stage, risk, molecular results and recovery.
- Treatment details
- China's 2023 guidance examples include CapeOx (capecitabine and oxaliplatin) and FOLFOX (oxaliplatin, fluorouracil and leucovorin). One cycle count cannot apply to every patient.
- Cycles and timing
- The guidance lists starting at about 4 weeks post-operatively and courses of 3 - 6 months, adjusted for poor recovery. Low-risk stage Ⅲ colon cancer (T1 - 3, N1) may discuss 3 months of CapeOx. These are conditional guideline examples, not personal deadlines.
Ask separately about cycle visits, oral medicines, infusion pumps and reassessment. Report hand or foot numbness and diarrhoea promptly. Doctors decide dose reduction or delay.
Pre-operative radiotherapy and medicines for rectal cancer
Locally advanced rectal cancer at appropriate risk, jointly planned by surgery, radiation and medical oncology.
Pre-operative radiotherapy and medicines for rectal cancer
Locally advanced rectal cancer at appropriate risk, jointly planned by surgery, radiation and medical oncology.
- Treatment details
- Use long-course concurrent chemoradiotherapy or short-course radiation followed by chemotherapy. Total neoadjuvant treatment places planned treatments before surgery; it is not a single medicine name.
- Cycles and timing
- The 2023 short-course example is 5 sessions; long-course uses 25 - 28. At 5 sessions weekly, irradiation lasts about 5 - 6 weeks. Simulation, planning, interval medicines, reassessment and surgery are additional. Irradiation completion is not a return date.
Bowel, urinary, sexual and fertility effects are possible. Surgical timing depends on the whole plan. Do not book the entire trip yourself based only on short- or long-course labels.
Medicines and local treatment for metastatic disease
Assessment by metastatic sites, resectability, molecular results, prior medicines and fitness.
Medicines and local treatment for metastatic disease
Assessment by metastatic sites, resectability, molecular results, prior medicines and fitness.
- Treatment details
- Discuss chemotherapy with relevant targeted medicines. Eligible dMMR or MSI-H patients may discuss immunotherapy. Surgery, ablation or radiation for liver or lung metastases needs assessment with systemic treatment.
- Cycles and timing
- If pre-operative medicines may enable resection, agree reassessment beforehand. Long-term medicines continue, change or switch according to efficacy and side effects, without adopting adjuvant chemotherapy end dates.
Written opinions should identify the goal: enabling resection, controlling disease or relieving symptoms. US-listed medicines may lack the same Chinese indication or availability.
04
When to discuss a second opinion or MDT
What to discuss after a recommendation for a permanent stoma
Submit pelvic MRI, location and the existing plan to colorectal surgery and radiation teams. Compare safe margins, pre-operative opportunities and function. Ask for written sphincter-preservation criteria, risks and expected function.
With liver metastases, treat bowel or liver first?
Review primary symptoms, all metastases and liver status together, involving colorectal and liver surgery, medical oncology and relevant intervention or radiation teams. Sequences differ in admissions and budgets.
If no tumour is visible after chemoradiotherapy, can surgery be omitted?
Watch-and-wait is only suitable for specially assessed rectal cancer patients, with complete clinical response assessment and ongoing monitoring. Symptom resolution or one improved image is insufficient. Discuss follow-up burden and local regrowth management too.
05
New medicines and other treatment options
Immunotherapy does not apply to every colorectal cancer
Check MMR or MSI first, then phase, current Chinese indications and regimen. Individual studies omitting surgery after immunotherapy cannot directly apply to MSS patients or replace surgical assessment.
Would ctDNA or new targeted medicines change the current decision?
Circulating tumour DNA (ctDNA) is blood test information, not standalone proof of cure or a reason to stop medicines yourself. Ask its purpose and effect on plans. New medicines and trials also require targets, prior medicines, screening and self-pay scope checks.
06
Stomas, bowel function and daily life after returning home
Practise stoma care before discharge
Ask nurses to demonstrate changes, skin care and observation and let the patient or companion practise. Obtain suitable product specifications and purchasing sources, with hospital contacts for leaks, skin problems or substantial output changes.
Record bowel changes rather than adding antidiarrhoeals yourself
Surgery, radiation and medicines can all change bowel function. Record frequency, consistency, food and other symptoms for assessment. Significant diarrhoea, inability to drink or dehydration signs need prompt care without waiting for the platform.
Return handover includes function and further treatment
Keep stoma instructions, radiation summaries and follow-up plans alongside pathology and medicines. Report numbness affecting walking or daily tasks. Doctors assess return travel and fitness to fly based on recovery.
07
International department costs and budgeting
This page uses self-pay pricing for international departments / international medical services at Chinese hospitals. A verifiable published total price for a full course of care for Colorectal cancer has not yet been obtained. Ordinary outpatient, standard ward or insurance settlement amounts cannot be used as substitutes.
Available international department itemised charges
The following charges come from the International Medical Center at The University of Hong Kong-Shenzhen Hospital, published on 2025-06-04, using the amounts converted on 2026-10-12. They are references for itemizing a budget, do not represent prices at all hospitals and are not a package for Colorectal cancer.
| Item and scope | Reference amount (USD) |
|---|---|
| Specialist outpatient consultationPer consultation, with fees varying by doctor category. Tests, medicines and treatment are charged separately. | USD 90 - 300 |
| Adult inpatient consultationDaily consultation charge only. Beds, surgery, medicines and other charges are separate. | USD 150 - 220 |
| Single standard room bedPer bed per day, corresponding to Standard Room 1 and Standard Room 2 in the published schedule. Inpatient consultations and treatment are excluded. | USD 180 - 220 |
Converted using the reference exchange rate on 2026-10-12: USD 1 = CNY 6.6928, rounded to USD 10. Dollar amounts are for budgeting only. The hospital bills in its actual settlement currency; exchange rates and payment fees are separate.
Specify stomas and reversal in surgical costs
International department quotes should list procedure, anaesthesia, pathology, staplers and other consumables, rooms, stoma care and complication boundaries. Quote later reversal separately. One operation estimate is not a whole course.
Separate adjuvant and metastatic medication costs
Estimate adjuvant treatment by planned duration, cycle medicines and reassessment; metastatic treatment by current phase and next assessment. Imported or Chinese-developed medicines, targeted combinations, pumps and line placement may affect costs.
Separate rectal radiotherapy and metastatic lesion management
Explain simulation, planning and irradiation separately. Liver surgery, ablation and other local treatment have separate admission and reassessment costs. Stay duration includes intervals and recovery. Travel is separate from hospital charges.
SinoCareHub helps obtain an itemised estimate from the prospective receiving international department, specifying the plan, date, validity period, included items and separately charged items. Hospital medical charges, platform coordination fees and third-party costs such as transport and accommodation are explained separately. The budget is updated if the treatment plan changes.
08
Which records to organise first
- Colonoscopy and full biopsy or resection pathology, including post-operative margins and nodes.
- Available chest and abdominal imaging; pelvic MRI for rectal cancer and original liver metastasis images.
- Available MMR, MSI, RAS, BRAF and other molecular testing, with samples and dates.
- Surgery, stomas, each medicine line, radiation plans and summaries, responses and main side effects.
First submit the records you already have and identify anything missing. The prospective receiving hospital decides whether additional tests or borrowing slides or paraffin blocks are needed for this assessment.
09
Common patient questions
Can every temporary stoma be reversed?
Not necessarily. Surgeons assess anastomotic healing, further treatment, health and function. Obtain expected assessment points and budget another visit and possible admission.
Does robotic surgery make sphincter preservation easier?
Equipment names alone cannot determine this. Sphincter relationships, margins and function matter more. Ask why the surgeon proposes this approach and what alternatives exist.
Can I choose three rather than six chemotherapy months myself?
Do not shorten treatment yourself. Guideline durations depend on stage, risk, combinations and tolerance. Ask for the basis and discuss side-effect adjustments, not choices solely for travel convenience.
Does normal CEA exclude recurrence?
One tumour marker is insufficient. Doctors assess symptoms, imaging, endoscopy and trends. Do not cancel scheduled reassessment because a value is normal.
