When does a teenager with scoliosis need a brace?

Braces mainly help limit curve progression in young people who are still growing. Doctors consider the curve on standing X-rays, change over time and skeletal maturity before recommending observation, bracing or another treatment.

The angle alone does not settle it

For adolescent idiopathic scoliosis, a brace mainly limits progression during growth. At the same angle, a teenager growing rapidly and one nearly at skeletal maturity can have different risks of progression. Older images show whether the curve has been increasing.[1,2]

The Cobb angle measures the curve on an X-ray. Bring dated standing whole-spine images and reports so the doctor can compare the same curve and consider skeletal maturity.[1,4]

When is observation enough?

Beijing Chaoyang Hospital's bone disease centre describes a staged approach based on angle and change over time. A small, stable curve may be monitored. Bracing is more often discussed when growth remains and the curve is substantial or at risk of progression. A large or progressing curve may need a surgical assessment.[1,2]

Typical angle ranges differ across professional sources. If opinions differ, ask whether the doctors agree on how much growth remains, where the curve is and whether it has changed.[1,2]

A brace needs follow-up after fitting

A brace is fitted to the body and the location of the curve. Its usual aim is to keep the curve from worsening during growth. The doctor and orthotist check the fit and decide whether it needs adjustment as the teenager grows.[2,3]

If considering a brace in China, organise previous images and measurement dates. Ask the treating team who will handle fitting, adjustments and follow-up after return. Before leaving, ask for the brace type, individual wearing plan and which images to review.

If a local team will take over, use the guide to follow-up after returning home to organise the records so the same curve can be compared.
